If you are a primary care veterinarian, learn how to transfer a patient to our care, and and what to expect throughout the process.

Step 1: Complete Online Transfer Form
Complete our online transfer form below. The form will allow you to upload any medical notes, radiographs, bloodwork, etc.

Step 2: Call our ER Veterinarians
Once the form has been submitted, referring veterinarians are encouraged to call us to discuss the case and goals of transfer with one of our ER Veterinarians.
Our doctors will be happy to help stabilize critical patients and assist with diagnosis, treatment plans, or any other questions you may have. This will open up the line of conversation between referring veterinarians and our ER department for a smoother transfer and building a strong relationship.
At this time, we can also discuss a rough estimate to relay to clients, setting up realistic expectations prior to transfer.

Step 3: Sharing Patient Records
At the end of your patient's visit, we will send you our patient records for outpatient services.
If your patient is hospitalized with us, we will send you record updates every morning, and once all records are completed upon discharge. If there are any pending blood work, cytology, radiology report, etc, these will be sent as soon as they are received and reviewed.
We will also call the pet owners with the results of the tests we sent out unless otherwise specified by referring the veterinarian.
